Learning Lab: Changing the Out in Outcomes

This session is designed to address the impact of performance improvement initiatives on patient/client outcomes. Utilizing real day examples, we will walk through strategies to address gaps in care which impact these anticipated outcomes. Multiple setting providers have an obligation to provide the best care possible, and although incidences occur, providers should be cognizant of the need for preventive strategies.

Describe performance improvement (PI) process.
Identify the benefits of PI strategies to impact/improve outcomes.
Apply PI strategies in daily operations.

About the Presenter:
Dr. Smith is the regional director for quality and clinical transition for Community Health Systems (CHS), Inc., based in Franklin, TN. CHS, Inc., currently owns and operates 195 hospitals in 29 states, and Thomas has responsibility for hospitals predominantly in the states of Florida, Georgia and South Carolina. Thomas currently oversees clinical transition and care management, and assists the facilities with following State and Federal regulations. Working with the administrative teams, Thomas provides oversight and feedback on internal processes, including assistance with performance and process improvement. Thomas is the former President of NAHQ and has been active in the Virginia State Association. Dr. Smith has over 35 years of experience in nursing, and also serves as adjunct faculty for masters programs in nursing and healthcare administration.
