HQ Insights: Online, On-demand Competency-based Continuing Education for Healthcare Quality Professionals
This program is designed to help you apply skills aligned to the Healthcare Quality Competency Framework. Each topic contains three curated sessions that offer practical guidance from healthcare quality experts. Upon completion of the evaluation in your My Learning, you will receive three CPHQ® continuing education credits.
These sessions focus on evaluating and improving healthcare processes and care transitions to advance the efficient, effective and safe care of individuals and defined populations. Specifically, population health and care transitions:
- Enhance your understanding of populations your organization serves.
- Focus on the care continuum and the areas of care necessary to support populations.
- Facilitate analysis of populations and communities and identifies resources for specific populations.
- Incorporate public health approaches into the care model.
After these sessions, you will be able to:
- Identify the post-acute care quality metrics that influence hospitalization and re-hospitalization and discover initiatives that have a positive impact on hospitalizations.
- Achieve improved transparency of outcomes by using PQIs and other measures.
- Improve coordination outside the hospital by working with community leaders and health plans.
- Work toward improved population health by addressing the roles of clinicians within the hospital and involving community and health plan leaders in improving PQI and other measure results.
- Identify how the Patient and Family Advisory Council (PFAC) increases engagement, improves the experience for patients and caretakers, and contributes to overall quality improvement.
